What medical weight loss actually is.

The plain version first: medical weight loss treats the metabolism behind the weight — thyroid, insulin, hormones — with a supervised plan that may or may not include a GLP-1, dosed and monitored on real data. Three terms get blurred together in the ads, and the difference is the whole point.

Who qualifies

Is a medical weight-loss program right for you?

This isn’t decided by a number on the scale. It comes from what your metabolism is doing and what you’ve already tried — but here is the honest shape of who it tends to fit.

Likely a fit if

  • You’re doing the work — eating well, moving — and the weight won’t move.
  • You’ve regained after a diet, or after stopping a GLP-1, and want to understand why.
  • You’re on a GLP-1 already and want to protect muscle and do it with real data.
  • You want the metabolism tested — thyroid, insulin, hormones — before or alongside any medication.

Not the right starting point if

  • You’re pregnant or breastfeeding — this isn’t the right time for a weight-loss protocol.
  • You want a GLP-1 mailed to you with no testing or follow-up; that isn’t how it’s prescribed here.
  • You have an eating disorder history that needs specialized care first — we’ll say so.

How is this different from a commercial weight-loss program?

Commercial programs hand everyone the same plan. We start with labs, continuous glucose data, and body-composition scans, then build your plan with you around what your metabolism is actually doing — with a medical provider adjusting it as your numbers change.

What does the weight-loss program cost, and what comes with it?

Total Health Coaching is $799 for three months and ten appointments with your provider. That includes two full metabolic lab panels, a Lingo continuous glucose monitor, a Keto-Mojo ketone and glucose meter, and two InBody body-composition scans. Supplements, additional labs, and extra monitors are not included. Membership is not required — though Optimization members start at $100 registration plus $499 for the same three months. Everyone begins with the $299 Total Health Assessment, credited in full toward the program.

Will I lose muscle along with fat?

That is exactly why we scan body composition rather than tracking scale weight alone. Protecting lean mass while fat comes off is part of how the plan is built and monitored.

What are GLP-1 medications like semaglutide and tirzepatide, and do I need one?

GLP-1 receptor agonists — semaglutide and tirzepatide are the two most discussed — mimic a gut hormone that slows stomach emptying and reduces appetite signalling. They can be genuinely effective, and they are not a metabolic diagnosis. If your thyroid is under-treated, your insulin is high, or your sex hormones are off, that is still true on a GLP-1 and still true after it. We would rather show you what your labs say first, so that whatever you decide about medication is an informed decision rather than a default one.

I am already on a GLP-1. Can you work with that?

Yes, and it is a good time to have real data. The most common problems we see on a GLP-1 are muscle loss from rapid weight loss and eating too little to hold onto lean mass. Body-composition scans show whether what you are losing is fat or muscle, labs show what your thyroid and insulin are doing underneath, and your provider builds the protein, training and nutrition side around the medication you are already taking.

What are the downsides of GLP-1 medications?

The common ones are nausea, constipation and reflux, especially while the dose is increasing. The one people underestimate is body composition: fast weight loss without enough protein and resistance training takes muscle with the fat, which lowers your resting metabolic rate and makes the weight easier to regain. Appetite also tends to return when the medication stops, so what happens next depends on whether the underlying metabolic picture was ever addressed. That is the part we can measure and work on with you.
